A client's verbal assurance is useful discovery input but it is not the same as evidence. Ask what artefact demonstrates the claim: policy, contract, log, model card, test result, approval record, meeting decision or system configuration.

Record where evidence came from, who provided it, when it was reviewed and whether it fully supports the claim.

If evidence is missing, mark the status as an evidence gap rather than treating the control as complete.

  • •Claim -> evidence -> owner -> date -> conclusion.
  • •Missing evidence is a finding, not an invitation to assume.
  • •Keep rationale reproducible for another consultant.
